Discount Plan Guidelines

• Family plans consist of spouses and/or children under the age of 20.
• Cleaning included is a simple cleaning and cannot be substituted for patients with Periodontal disease. Additional savings are available for patients needing Debridement, SRP, or other Periodontal Therapy services.
• Benefit coverage is effective for participants who remain enrolled in their plan until the completion of treatment. If enrollment expires before treatment is completed and the participant does not re-enroll, coverage is no longer available.
• Patients are eligible for re-enrollment annually.

Plan Exclusions and Limitations

The Bella Smiles Savings Plan does not constitute an insurance plan. It has been intended to provide our customers access to essential dental treatment at the most economical pricing.

The savings plan:

• Is a standalone coverage plan not to be combined with insurance benefits.
• It cannot be used in a workman’s compensation treatment plan.
• Is for use in dental treatments that lie within our realm of expertise.
• Cannot be used for dental treatment with another dentist or specialist to whom we refer.
